Purpose: Major Windows documentation cleanup

Description:
Bring the Windows h4h5 documentation up to date for the recent project changes, and in general.

Note:
Building debug h4toh5 libraries on Windows is now considered "optional", because we don't provide HDF5 debug binaries to link with.  Therefore, users will need to also build HDF5 on their own.
