Description:
    Add note that either creation order tracking 
        or creation order tracking and indexing may be set.
    Add note that if an index is to be built, it must be specified at 
        creation time.  HDF5 currently provides no mechanism to turn on 
        creation order tracking at creation time but build the index later.
