[svn-r224] Initial implementation of the upper levels of PHDF5.  The
MPIO lower interface layer (H5Fmpio.c) has been commited by Kim already.
All PHDF5 codes are "bracket'ed" by #ifdef HAVE_PARALLEL macro.
