Merge pull request #1696 in HDFFV/hdf5 from ~NFORTNE2/hdf5_naf:hdf5_1_8 to hdf5_1_8

  Add fixes that were mistakenly omitted from merge of pull request 1594 to 1.8 branch.
  Fix issue with direct chunk write not updating the "last chunk" index cache.  Fix issues involving datasets being "not allocated" when they contain cached raw data.
