Merge changes from Coverity branch to trunk:

Original issue was #338.
Refactored code to clean up allocations consistantly, completely, check all returns, assign initialization values and handle errors.
Tested on:
MacOSX/64 10.9.3 (amazon) w/gcc 4.9, FORTRAN, C++, parallel & multi-threaded
(too minor to require h5committest)
