Revisions based on feedback from [partial] code review of SWMR merge to trunk: - Remove #ifdef'd code from H5ACproxy_entry.c - Add errors and tests for preventing R/O protected entries from moving - Updated asserts for "allow last pins" flag when flushing & invalidating the cache
