Review and cleanup page buffering code, along with propagation of corresponding
changes to passing DXPLs around internally, so that we have the correct kind
of DXPL (metadata vs. raw data) when pages in the page buffer need to be
evicted (which doesn't always match the type of page being inserted into the
page buffer list).
