H5Fcreate and H5Fopen:
Clarified behavior and interaction of flags.
Did not add further documentation of H5F_ACC_DEBUG as that
flag is scheduled to be removed from the library.

Added text explaining that H5F_ACC_RDWR and
H5F_ACC_RDONLY are not relevant in the case of H5Fcreate.
