RM/H5D/H5Pset_fapl_mpio & H5Pget_fapl_mpio
Updated H5Pset_fapl_mpio & H5Pget_fapl_mpio descriptions, adding
emphasis that the MPI communicator and Info object in these functions
are duplicates of the stored information rather than pointers.

Updated some of the HTML coding.
